ALL employees and outside visitors are required to complete and submit the below COVID-19 self-screening form before being able to enter any of our three office locations. The form must be completed and submitted on the morning of your intended office visit.
Once the form has been submitted you will receive an approval-to-visit message or a denial-to-visit message, depending on your responses. If you are denied visitation based on your self-screening responses, please contact our front desk at (585) 232-5300 to reschedule your appointment for a later date.
